- Call the office to schedule a follow-up appointment (321) 237-1552. If we are assisting other patients at the time you call, and you receive voicemail, please leave a voicemail with your full name, phone number, and what you are requesting, and staff will get back with you as soon as possible.

How do I schedule a follow-up appointment?
You can schedule a follow-up by calling our office directly, sending a secure message through your Charm patient portal, or scheduling through the link just above.
What if I need to reschedule my appointment?
Kindly reach out to our office by phone (321-237-1552) at least 48 business hours before your scheduled appointment time to reschedule your appointment. Please note that a $100 fee will apply if you miss your scheduled appointment without notifying us at least 48 business hours advance. For example, for a Monday appointment one must cancel of reschedule Thursday, and for a Tuesday appointment one must reschedule on a Friday.
This ‘no-show/late-cancellation’ policy fee allows for the practice to continue to operate, given the high costs of overhead, and the detriment that no-shows can pose to a medical practice’s financial health. Additionally, ‘last-minute’ cancellations remove the ability the practice would have had to be able contact and schedule a patient from the waiting list into that spot. Thank you for your understanding of the importance of the ‘no-show/late-cancellation’ policy.
Can I discuss new symptoms during a follow-up?
Yes, follow-up appointments are a great opportunity to discuss any new or ongoing concerns.
What should I bring to my follow-up visit?
Please bring a current list of medications and any relevant health updates for review. Fill out any necessary paperwork before your scheduled appointment time, such as the questionnaires about your symptoms that your doctor sends you before your appointment. Also, update any insurance changes with the administrative team before every appointment.
